Submit forms

If you would like to submit your supplier forms via our portal, please follow these steps:

1. DOWNLOAD the Supplier Forms you wish to submit.
2. COMPLETE the supplier forms offline and re-save to your computer.
3. Go to FILE UPLOAD page to UPLOAD your supplier forms and attachments to send to MGM Brakes.
4. Supplier forms can still be emailed to your MGM Brakes contact at their email address.
